BizTalk will not handle this in any sort of automatic way. The 837 XSDs will give you clues about what qualifiers are valid for a particular field, but they do not get set on those fields unless you set them in the map - either in the Value property of the destination node or via the output of a link (from a source node or functoid). You should not modify the XSD unless you need to support a non-standard qualifier that you and your trading partner have agreed to use - but you should stick to the standard qualifiers and encourage/require your partners to do so as well to avoid the need of such customization, and if you do make such modifications it should be to a trading partner version of the schema that gets properly mapped to a canonical format that does use the standard codes.
To further clarify, if you need to set the primary diagnosis code to BK and set other diagnostic codes to ABK, you have to provide that output from the map. You also have to ensure that you link to the proper HI node - only the first HI node for the Primary Diagnosis will accept BK as the qualifier (per WPC standards); subsequent ones can have ABK. I've written a couple blogs on this topic here and here.
What you really need here is to review the WPC specification for the Professional Claim v. 5010 and your trading partner's companion guide for the claim. These will provide all of this information so you can do your mapping correctly. You will also very likely need to work with an EDI claims specialist to get this right - HIPAA transactions are particularly challenging, and the claim forms are probably the most complicated of them.

Have an issue where I am trying to debatch a flat file in BizTalk Server (comma delimited to tab-delimited) into individual flat files based on a value (in this example it would be PONumber) in the original file.
Sample input:
PartNumber,Weight,PONumber,Other

This would outcome into 2 messages such as:
PartNumber Weight PONumber Other
21519 234 46788 1
and
PartNumber Weight PONumber Other

I have seen similar things but no definite answers, or samples are dead links. Does anyone have a sample where they have done something like this or have a good solution?
Option 1: Convoy pattern
Change your schema so that it has a max occurs of 1 for the PO line, this will debatch each line into it's own messages when it is received.
Promote the PONumber so that it is a promoted property in the message context.
Have an Orchestration that has a correlation set based on the PO number, and initialises this on the first receive shape.
Have a receive shape with a following correlation that is in a wait shape inside a loop to receive all the other lines with the same PO number and combine them into a single message.
Option 2: Staging database
The other option is to just insert all of the rows into a SQL database, and then have a stored procedure that you poll that gets all the lines for a single PO.
This can sometimes be simpler, and avoids the issue of Zombies as you can implement this as a messaging only pattern or using a simpler Orcherstration without a loop.
I am using BizTalk 2016 with Feature Update 3 (CU7), and the BizTalk Server Administration Console version 3.12.774.0
In the BizTalk Group I go to the Parties Node
Select a Party and go to it's Agreement in the Agreements list
Open the Agreement and go to the second tab (outgoing settings e.g. BizTalkApp->ThirdParty)
Go to Transaction Set Settings -> Envelopes
There is one envelope record. Go to this an change one of the values, e.g. GS4 - change from CCYYMMDD to YYMMDD
Click Apply
BizTalk displays the error Object reference not set to an instance of an object. (Microsoft.BizTalk.Administration.EdiText)
You cannot apply any changes to Envelope GS values because of this error. Changes to other agreement properties such as Interchange Settings -> Identifiers can be saved fine.
Has anyone come across this error before? How can we get past it?
It turns out that this error was caused by the fact I didn't have any value entered in the GS1 dropdown. Once I had entered this value in the row then other changes to the row could be saved.
Biztalk suspends instances of messages where the format or value of GS segments do not match those specified in the envelopes tab. So this means that I will have to analyse all the EDI documents we are receiving from our external party and make sure the GS1 values they use are covered. If there are more than one type of GS1 value used I will have to enter multiple rows in the Envelopes list.
I have an exercise I'm working to complete; previously it was de-batching multiple XML messages from one file into individual files. Then I had to route individual files based on a field value which had been promoted using filters on a port. Now the exercise has evolved into taking a multi record XML file, breaking it down to individual XML records, and routing their output to different folders based on a value in one of the fields. The hurdles are as follow:
I can't promote a repeating field such as the one I have to use to sort the outbound messages
The value of the field is a system.int32; I am sorting on a "equal to or more than 900" and "less than 900" so I need the int type.
Beyond simple "idNUm >= 900" I am in over my head with the necessary expression(s).
I have the basic orchestration design down, I am just lacking the expressions. The node I am looking to validate against is IDNum, and occurs in each record.
UPDATE: Still not working
I put in the following in my expression: IDNumDefined.Customer.IDNum >= 900
and I get "identifier Customer does not exist in "IDNumDefined"; are you missing an assembly reference?" and "unexpected token '>=' "

The debatching has to occur using an Envelope and Body schema.
Once you have this figured out, the debatching can occur using a simple XML disassembler. In the body schema you can quick promote your idNum field by associating a PropertySchema with it.
Once this is taken care of, it is easy to use 2 send ports in order to set your filter subscription(s).
I have a requirement to load the csv into DB using oracle apex or pl/sql code, but the problem is they are asking to load the csv file which will not come with same number of columns and column names .
I should create table & upload data dynamically based on the file name and data that i'm uploading.
For every file i need to create a new table dynamically and insert data that are present in csv file.
For Example:
File1:
col1 col2 col3 col4 (NOTE: If i upload File 1, Table should be created dynamically based on the file name and table should contain same column name and data same as column headers of csv file . )
file 2:
col1 col2 col3 col4 col 5
file 3:
col4 col2 col1 col3
Depending on the columns and file name i need to create table for every file upload.
Can we load like this or not?

answer is buried in here somewhere... I introduced some considerations
and assumptions you will need to think about before going into the
task. In the end, you'll find that Oracle APEX actually has a
built-in solution that satisfies exactly what you've specified... with
some caveats.))
If you are working within the Oracle APEX platform, you will have some advantages. APEX Version 4.2 and higher has a new page element called "Data Loading". The disadvantage however is that the definition of the upload target is fixed and not dynamic. You will need to know how your table is structured prior to loading the data.
One approach to overcome this is to build a generic, two-column table as your target, which will serve for all uploads. Column 1 will be your file-name and column two will be a single clob data type, which will contain the entire data file's contents including the header row. The "Data Loading" element will give the user the opportunity to verify and select this mapping convention in a couple of clicks.
At this point, it's mostly PL/SQL backend work doing the heavy lifting to parse and transform the data uploaded. As far as the dynamic table creation, I have noticed that the Oracle package, DBMS_SQL allows the execution of DDL SQL commands, which could be the route to making custom tables.
Alex Poole's comment is important as well, you will need to make some blanket assumption about the data type or have a provision to give more clues about what kind of data is contained. Assuming you can rely on a sample of existing data values is not good... what if all the values in your upload are null? I recommend perhaps a second column in the data input with a clue about the type of data for each column... just like the intended header names, maybe: AAAAA = for a five character column, # = for a numeric, MM/DD/YYYY = for a date with a specific masking.
The easier route:
You will need to allow your end-user access to a developer-role account on a workspace of your APEX server. It is not as scary as you think. With careful instruction and some simple precautions, I have been able to make this work with even the most non-technical of users. The reason for this is that there is a more powerful upload tool found under the following menu item:
SQL Workshop --> Utilities --> Data Workshop
There is a choice under "Data Load" --> "Spreadsheet Data"
The data load tool will automatically do the following:
Accept a CSV formatted file through a browse function on your client machine
Upload the file and parse the first record for the column layout (names)
Allow the user to create a new table from the uploaded file, or to map to an existing one.
For new tables, each column data type can be declared and also a specific numeric/date mask if additional conversion from the uploaded data is necessary.
Delimiter type, optional enclosures (like double quotes), decimal conventions and currency types can also be declared prior to parsing the uploaded file.
Once the user has identified all these mappings and settings, the table is created with the uploaded data. Any errors in record upload are reported immediately afterwards with detailed feedback on the failed records.
A security consideration to note:
You probably do not want to give end users access to your APEX server's backend... but you CAN create a new workspace... just for your end users... create a new database schema for receiving their uploads, maybe with some careful resource controls. Developer is the minimum role needed... but even if the end users see the other stuff there won't be access to anything important from an isolated workspace.
I have implemented the isolated workspace approach on a 4.0/4.1 release APEX platform a few years back, and it worked nicely. Our end user had control over the staging and quality checking of her data inputs (from excel spreadsheet/csv exports collected from a combination of sources). I suppose it may have been even better to cut her out of the picture entirely and focused on automating the export-review-upload process between our database and her other sources. In this case, the volume of data involved was not great enough (100's to 1000's of records) and the need for manual review and edit of the exported data was very important prior to pushing it into the database... so the human element was still important in this case - it is something you'll want to think about now.

I also have tried to find any reference for the tab-delimited report types, but it seems to be scattered all around the web. The best description I found was part of the instructions for the Amazon Inventory Loader. (Note: may require a MWS seller login, the corresponding XLS does not have all columns described on the linked webpage) That page should answer most of your questions.
Since the link above might require a login, here's a short description on what these columns do:
asin1 refers to an item's Amazon Standard Identification Number. Every item on Amazon has such a number, there even is a Wikipedia entry describing what it is.
product-id along with product-id-typerefers to the item's non-Amazon standard identification number, if such a thing exists (otherwise it'll contain a copy of the item's ASIN).
product-id-type=1 -> product-id is ASIN
product-id-type=2 -> product-id is ISBN.
product-id-type=3 -> product-id is UPC
product-id-type=4 -> product-id is EAN (now called GTIN)
sku is your own item identifier such as part number. You created the link between an ASIN and your own SKU by creating the product. (I know you didn't ask for this, but this is for the sake of completeness)
listing-id There does not seem to be a lot of documentation on what theses are. There is a page explaining how to find out an item's listing id. It does not say why you'd ever want to know, though. I assume a listing ID identifies a certain seller's (your) offer for a specific item, but all MWS requests I've ever done either required me to link to a ASIN or my own SKU, but there may be others that require this id.
Sidenote: I find it weird that a single listing-id may relate to more than one ASIN - otherwise, why are there columns named asin2 and asin3?
